Distance Testing Instructions
After booking your appointment, send samples to our office no more than one week prior to your session and complete the intake forms.
- Hair Collection: Cut a small amount of hair close to the root from the neck area. Place in a labeled plastic bag.
- Saliva Collection: Swab the mouth and cheek with a cotton swab and place in a labeled plastic bag.
- Forms: Complete the intake and disclaimer forms before your appointment.
- Mailing: Send samples via USPS Priority Mail within 7 days of collection.
